First, mail us or drop off at our office a photocopy of your tax return statements (can be form W-2, W-4, 1099, 1040, 1065, 941, etc.) and you must include an attached sworn affidavit statement from the holder (you) that has been properly notarized by a Texas notary public (you cannot use a notary public from any other state or country) for each year of tax returns and/or each separate tax return statement you have.
For example you could easily have two or more different 1040’s in a year and each individual 1040 would need to have it’s own notarized and signed by you certified copy affidavit and apostille or authentication certificate issued to it.

The Texas Secretary of State office will reject any non-recordable document (tax return statement copies) if the notarization was not completed by a Texas notary public and if the tax returns holder does not sign their name and a certified copy affidavit statement on the notarized photocopy like the one below.
Either you, an attorney, or a Texas notary public can copy/paste this below notary jurat or acknowledgment verbiage (only choose one set of verbiage not both) onto a blank Microsoft Word document (DO NOT print this whole webpage out as is and get it notarized) and attach it to your tax return statements or simply handwrite (print NOT cursive) this state-required notary jurat (first one) or notary acknowledgment (second one) verbiage onto the backside of your tax return statement copies.
